The Olympia CUSD 16 School District will offer sports physicals at the District Office (903 E 800 North Rd, Stanford) on Thursday, July 13th. The cost of the physical is $20.00. The event will take place from 8:00am to noon with walk-ins welcome!
Please bring a completed consent form and IHSA form with you when you come.
